Free Travel Paper Pass
If you lose your paper Free Travel Pass or if it is stolen or damaged, you should complete the FT PSC 1 form. If you are not registered for a Public Services Card Free Travel, you will receive a letter inviting you to make an appointment to register. You will not receive a Public Services Card Free Travel until such time as you have successfully registered. You can get an application form:

If you wish to replace a lost/stolen/damaged Public Services Card Free Travel, you must telephone 1890 837000 to request a replacement
