e-form applications are no longer available on www.welfare.ie for the following services- State Pension Contributory, Widow's, Widower's or Surviving Civil Partner's (Contributory) Pension, Child Benefit and Jobseeker's Benefit.

The old Adobe e-forms for the schemes below have been retired due to their very low usage and poor customer experience.

To apply for the following schemes you can either download a paper form from the links below or pick up a form from your local Intreo Centre.

Paper application forms can be handed into your local Intreo Centre or Branch office or posted to the relevant section of the Department.

MyWelfare is a new web site built by the Department for customers so that you can conduct your business with us on line.  Currently customers can avail of these on line services:

  • Apply for Jobseeker’s Payment, Paternity Benefit, Maternity Benefit, PRSI Refunds
  • Apply for a Payment Statement or a Contribution Statement
  • Close a Benefit Claim
  • Apply for Jobseeker Holiday
  • Submit Work and Skills Information
  • View / Schedule Appointments
  • Update your Address
