As you know, applications for entitlements under the Insolvency Payments Scheme are being administered on a new IT system by the Department of Social Protection from Monday, 3rd October, 2011.
Where you, as an employer representative, are submitting an application, it is important that full details have been provided in order to set you up as an employer representative on our system, so that the application can be dealt with speedily.
It has come to our attention that the details of some employer representatives have not been submitted to this Department.

- Employer Representative company name and address
- Employer Representative PAYE number
