Customers who do not receive an automatic payment will be required to complete an application form which will become available after 30th June 2011 from the following sources.
Completed application forms together with supporting documentation should be returned to
Department of Social Protection,
PO Box 131,
Letterkenny,
Co.Donegal.
It may take a number of weeks to process applications.
