If you lose your Free Travel Pass or if it is stolen, you should apply for a replacement Free Travel Pass by completing form FT 27. You can get an application form:

on the Internet at 'www.welfare.ie'
from your local Social Welfare Office
by contacting the Free Travel Section at the address In Section 3
